A polymyxin, and one of the last line options against multidrug resistant Gram negative organisms including carbapenem resistant Acinetobacter, Pseudomonas and Klebsiella.
These are older molecules that returned to routine use because resistance caught up with everything newer. Use is concentrated in intensive care and is almost always on the advice of a microbiologist. Colistimethate is a prodrug and is dosed in international units, which is a common source of confusion; the unit on the label should always be read carefully.
